Pre- and immediately post-operative images of a very severe cleft palate.

A standard unilateral reflection technique with preserved dorsal palatine artery would have been insufficient to close the deficit, and a bilateral sliding procedure would also have fallen short of creating closure.

Therefore, reflective flaps were created bilaterally and sutured in the midline.
